Please help with form 2441

I keep deleting form 2441 but it keeps coming back. And I cannot remove the $5000 entry on line 12.

If you are not claiming a Credit for Dependent Care, go to Deductions & Credits > Dependent Care Credit. Click Edit/Add.

Say YES to 'Do you want to review your dependent care info?'  In the dropdown menu, change dependent name to blank or No Entry.

Close TurboTax, clear your Cache and Cookies and re-open TurboTax.

Then, go to Tax Tools > Tools on the left-side menu.  Choose 'Delete a Form' from the pop-up menu.

In the list of forms in your return, Form 2441 should be deleted.
