Yes. You should have received a 1099-SA for any withdrawals. Check the website of your administrator to download Form 1099-SA. If you don't see it on the website, you will need to contact your administrator for the form.
The IRS requires you to prepare Form 8889 and attach it to your tax return when you take a distribution from an HSA.
HSA distribution aren't taxed that are spent on qualifying health expenses aren't taxed.
