My w2 says I had $52 of deductions from my paycheck for HSA contributions. However, I forgot to open the HSA so money was sent back to me. Do I need a corrected w2?

A corrected w2 would be the first place to start. I would talk to the payroll department first to clarify the amount that was deducted and then returned and see why the w2 came out with the wrong information on it. This is a strange error that is not generally seen in most companies.
