only out of pocket after any insurance reimbursement,  say you pay a doctor $100 and file a claim is filed with your insurance company. they send you a payment of $80 to reimburse you pursuant to your coverage, your deduction is $20.

Yes, you can only claim what you actually pay for. If your insurance reimburses you or pays for you, you did not really pay. You can, however, deduct your insurance premiums.
