First question is, "Did your employer remove the HSA contributions from Wages in boxes 1, 3, and 5?"
If your employer did so, then go to the HSA interview (Deductions & Credits->Medical->1099-SA, HSA, MSA...), and navigate to the screen that is headed: "Did your employer tell you about any other contributions?"
Ignore the comment about other years, and click on YES. Three new lines will appear, the bottom one being "Employer and payroll contributions not reported in Box 12 of your W-2". Enter the amount here; this will appear on the 8889.

in box 12 the employer should have combined any of its matching contributions with your own payroll deductions for the HSA. your contributions should have reduced lines 1,3, and 5 but not the employer's matching. if boxes 1,3,and 5 are wrong you need a corrected w-2.
thank you for answering so quick but the employer did not remove the HSA contributions from wages in boxes 1,3 and 5. he just contributes a monthly amount to the HSA plan.
