I received jury duty pay as well as mileage to the court on 1 check. My employer paid salary & requires reimbursement for days attended. Do I report mileage as income?

Report the total amount as jury pay—other income. Then report jury pay again in the amount of the reimbursement to your employer as a negative number. So you will only be taxed on the mileage.
