Yes, you must report money spent from your HSA in 2019 on your 2019 tax return.

 

If you spent money from your HSA in 2019 (presumably for qualified medical expenses on the debit card), then your HSA custodian (not your employer) should have sent you a form 1099-SA which reports on the amount distributed.

 

To get a copy, please contact your HSA custodian (the bank or financial institution that gave you the debit card) and ask for a copy of your 1099-SA.

 

NOTE: many HSA custodians make this form (and other tax forms like the 5498-SA) available online - look at the custodian's website and see if you can download a copy, or just call them.

You must report the distributions, and certify on your return that they were used for qualified medical expenses. If not, the IRS will send you a bill for tax and penalties.

 

Note that an HSA is not the same as an FSA (flexible spending account from a cafeteria plan).  You don't get paperwork for FSA spending and you don't report it on your tax return.
