The 5498-SA details the contributions made to your HSA.  Your payroll deferrals will appear on this form.

There's no reporting requirement for this form; it's for your records.  However, if you made after-tax contributions (not thru your payroll at work), then you'll enter those into TurboTax manually: Federal Taxes > Deductions & Credits > Medical > 1099-SA, HSA, MSA and follow the prompts to the screens after the 1099-SA input page.

You'll notice on the page for Let's enter your HSA contributions, your payroll deferrals from your W-2 already appear.

If you took money out of the HSA, then you should have received a Form 1099-SA, which you would need to enter in TurboTax (same pathway as listed above, on 1099-SA page).  Once you enter it, you'll be asked how much of the money was spent on medical expenses.

If you didn't take money out of the HSA and didn't receive a 1099-SA, there's nothing you have to do.

Do I need to amend my return or just add the new 5498-SA

Take a look at your W-2. If all of your HSA contributions were reported in box 12 with code W, there is no need to amend your return. The 5498-SA is just a verification of the contributions that were made to your HSA in the tax year.

If your contributions were not on your W-2 but were already reported on your return, again, you will not have to amend. In this case, you would see the contributions reported on line 12 of Schedule 1 of your tax return.

Should you have to amend, follow the steps below:

  1. Log into your TurboTax account
  2. Choose the tax year that you wish to amend
  3. Click Amend
  4. Make any necessary changes

Keep in mind that when you are amending, the refund tracker monitors the difference from your original filing to the amended return. It does not include any original refund or balance due. If you are claiming any previously unclaimed adjustments, you may see the difference as an additional refund amount.

To add the HSA contributions, under the Deductions & Credits menu, confirm the following:

  • Expand the menu for Medical
  • Click Start/Revisit next to HSA, MSA Contributions
  • Confirm the account ownership and click Continue
  • Continue in through the screens until you reach Let's enter your HSA contributions
  • Stop here. If all of your contributions were through payroll deductions and reported on your W2, do not enter anything on this screen. If that is the case, either leave the box empty or type $0 in the box next to Any contributions you personally made
