I contributed the maximum of $4150 to my HSA. $2800 is on my W2. The additional $1350 I just made. How do I know the full $4150 was deducted ($1350 shows on HSA summary)?

HSA Summary shows: Deduction $1350 Total Distributions $2394 Taxable distributions $0 Taxable earnings on excess contributions $0 Tax-free employer contributions $2,800 Excess employer contributions withdrawn $0 I just made the additional payment and entered it as made not through my employer. Is that correct?

Yes, it all seems to be correct.

The only amount that will be deducted explicitly on your tax return is the $1350 contribution that you made directly to the Health Savings Account (HSA) with your own after-tax funds.  The contribution made through your employer from payroll deductions or from your employer itself was made with pre-tax funds so you have already received a tax benefit for that contribution.

When you look at your tax return you should see your contribution of $1350 shown on Form 1040 Schedule 1 line 13.
