Form 8941 is "Credit for Small Employer Health Insurance Premiums."  (A link to the form is below.)

You do not say which software you are using, but if you did not pay premiums during your tax year for employee health insurance coverage through a Small Business Health Options Program (SHOP) Marketplace, it is likely you answered a question incorrectly in the Health Insurance section of your software.
