I was with my former employer from Jan- Aug 15th 2021, which was under HSA plan. Then I was hired by my current employer from Aug 17th - Dec 2021 and they use FSA plan, which I contributed to both. How do I file tax?

HSA contributions are reported on your Form 1040, but there are no reporting requirements for contributions to an FSA. According to IRS Publication 969, you are allowed to have both an HSA and an FSA in the same year.
