How do I report the child care costs when both dependents shared that same provider for 5 months? Thanks

You do not have to split the expense.  You can just enter the total for that provider under one child and then enter information on other providers.  You can claim a maximum of $3,000 of eligible dependent care expenses for one dependent and $6,000 for two or more dependents (total).
