HSA information is found primarily in three places:
1. On your W-2 in box 12 (a-d) you will probably see a code of W with a dollar amount. This is the sum of the HSA contributions made by your employer and by you through payroll deduction.
2. When you enter one or more 1099-SA forms that you get from the HSA custodian in the mail or online (note: if you had no medical expenses paid by the HSA last year, then you won;t get one).
3. If you made any HSA contributions directly to the HSA custodian, then look in your checkbook or other banking records to see what you contributed directly.
