You can find the 4852 form by following these steps:

1. Sign into your TurboTax Account
2. Enter your W-2 information
3. Click Continue> Do Any of These Apply To This W-2?
4. Check the box that says I DID NOT RECIEVE A W-2 FROM {EMPLOYER} AND NEED TO COMPLETE SUBSTITUTE FORM. 

• Even though you did receive a W-2 but it was incorrect.

Click Continue>YES

Proceed through the screens, entering the requested information to complete Form 4852.
