Form 8949 description of property inserts the name of brokerage firm with see attached statement. What is this attached statement?

Yes, if you have a lot of transactions, the entry will read like that because there's only 14 lines for short term and 14 lines for long term transactions on  Form 8949. 

 

If you imported, or entered more transactions than that, the program will create a statement with all the same details, as an attachment to the form. The information is basically what is on your 1099-B, with the possible exception of some basis information provided for noncovered securities.
