Form 8283

On Form 8283 Noncash Charitable Contributions I am being prompted for the following: Enter the entity name and identifying number from the tax return where the Noncash charitable contribution was originally reported, if different from above.

It then asks for the Name and identifying number. What are these and where do I find them? I have never seen this before.
