See if you have a continuation form. I get my W-2 electronically and also couldn't find the HSA contributions until I scrolled down to the bottom of the form. It seems that if you've more than 4 codes in box 12 you may get another form with the extra codes.

Box 12 of your W-2 is separated into at least 4 different parts as you discovered -- 12A, 12B, 12C, 12D. All of these are considered to be 'box 12'. As stated by user pk8, there may be multiple box 12 entries that you will need to enter.
As you are entering your information into TurboTax you will enter all of the entries shown in any of the box 12 areas. There will be a code and an amount to enter.
The entry with code W is your HSA contribution amount. When you enter that into your W-2, it will automatically be taken to the HSA section of your return. You will not need to enter it again in the HSA section.
