We switched to a healthcare sharing organization and didn't know we no longer qualified for the HSA. How do I report it on our return?

Did you have HDHP (High Deductible Health Plan) at any point in 2018? Don't count months in which you had the healthcare sharing organization coverage on the first day of the month, even if you still had the HDHP coverage.
If you had no HDHP coverage at any time of the year, then when you are asked in the HSA interview "Did [name] covered by a High Deductible Health Plan (HDHP) in 2018?", answer "no".
If you had HDHP coverage for at least one month in 2018, then when you are asked in the HSA interview "Did [name] covered by a High Deductible Health Plan (HDHP) in 2018?", answer "yes". This will expose three new lines. You will check the last one: "I had different plan types at different times of the year". The next screen will ask you for which month you had what: Family, Self, or None (None means any time you either did not have HDHP coverage or you had conflicting coverage).
You will get an error message about excess contributions. Withdraw whatever you can, and the rest is carried over to next year. You will be assessed a 6% penalty on the lesser of your excess carried over or the value of your HSA on 12/31/2018.
