We paid for childcare services in 2022. It was the same provider throughout the whole year. Same tax ID. 10months was at one of their locations and the summer camp was at another location same provider. Do we need to provide the address for both locations or since, it was the same provider with the same Tax ID, the address of one location is sufficient? Additionally, as we made our payments by credit card, there was a 3% credit card transaction fee added to the childcare fee monthly. Do we tally up the actual fee amounts all together before the credit card transaction fee or do we include the credit card transaction fee?

Not including the credit card service fee in your claim for child care in previous years will not be an issue.
In principle, if your child care is below the maximum allowable expenses ($3,000 for one child and $6,000 for more than one child, with the exception for tax year 2021 when the limits are respectively $4,000 and $8,000),you can amend your past tax returns to add that fee, but I think that it may not be worth the bother for such a small amount.
