Can I claim daycare expenses if the daycare is no longer in business

My daughter was enrolled in a daycare from January to June of 2016. They closed at the end of June and are no longer in business and I cannot reach the owner to get the tax form for my payments.

Yes you may.  You paid for the expenses.  However, if you do not have the EIN (the business's equivalent to their "Social Security Number"), then you may have to mail in this form in order to claim the expense.  If you don't wish to wait the time necessary to get back all of your refund (including the daycare) from a mailed-in return, you could do this:  e-file the return without the expense, and then amend the return to include the daycare.  An amended return must be mailed in, but if all it has is the daycare expense, it will be a small amount in comparison to the rest of the refund.

Here is an FAQ with links on how you can prepare the amended return:  https://ttlc.intuit.com/replies/3288565
