Box 12 of my W2 does not show any HSA contributions in 2018, but I received a 5498-SA from my employer that states I did make $2,675 in contributions in 2018. What to do?

Hi. You may have made the contributions yourself. If that's not the case, contact your employer & ask them to send you a corrected W-2.
