1040 sch 2 line 21 shows I have an additional tax amount from some other forms. It traces back to 5329-T. The only entries I have are some IRA amounts on line 16a and 24a.
If I understand it right these lines are compared to line 16 and 24 and the smaller is entered into line 17 and 25 which is used to calculate the additional tax.
My line 16 and 24 are zero and line 17 and 25 is empty.
Why am I getting an amount on 1040? It generates a penalty which then rolls to state which also penalizes me.

To clarify, on Form 5329 lines 4, 8, 17, 25, 33, 41, 49, 51, and 55 are empty but it shows an amount on line 8 of Schedule 2?
You didn't get a warning in the interview about a penalty (for example early distribution or excess contribution)?

Yes, there was a warning in the end. That is what triggered me to trace it back.
What did the warnings say?

After a little more digging found an errant entry on the IRA worksheet which was triggering the tax.
I have corrected that and it appears things are back to normal.
