Which forms do I need to mail for llinois returns (TurboTax Desktop)?

I e-filed my federal return, but I need to mail my Illinois (IL-1040) return because Illinois did not allow e-file (I did not file an IL return in the previous year).

 

I want to confirm exactly what documents I should include in the Illinois mailing packet.

Illinois packet currently includes:

 

  •     2025 Form IL - 1040
  •     2025 Schedule NR (Illinois Dept of Revenue)
  •     1040 U.S. Individual Income tax 2025.
  •     Schedule 1 - Additional income and adjustments to Income.
  •     Schedule 2 - Additional Taxes.
  •     Schedule 3 - Additional credits and payments.

 

  • W-2 (Kyndryl)

  • 1099-R

  • Payment check


My questions:

 

  1. Is the above list complete for Illinois filing by mail?

  2. Or do I need to include any additional federal forms such as:

     

      • Schedule B (Interest & Dividends)

      • Schedule D (Capital Gains)

      • Form 8949 (multiple pages)

      • Schedule E (Rental)

      • Form 4562 (Depreciation)

      • Form 8582 (Passive Activity Loss)

      • Form 8995A (QBI deduction)

      • Form 8960 (Net Investment Income Tax)

      • Form 5329 (Additional tax on retirement distributions)

      • Form 8889 (HSA)

     

  3. Or is including just Form 1040 + Schedule 1/2/3 sufficient?


Goal:

I want to include everything required, but avoid sending unnecessary worksheets or extra forms.

Only the Illinois forms you completed, any W-2's or tax forms with Illinois withholding taxes,

if there are amounts on line 5 of the Illinois 1040 then these additional ttchments

What must I attach to my Form IL-1040 when I subtract retirement income on Line 5?
You must attach a copy of your
● U.S. 1040 or 1040-SR, Pages 1 and 2 to support your subtraction for income from
■ qualified employee benefit plans, including 401(k),
■ railroad retirement,
■ IRA distribution,
■ converting a traditional IRA to a Roth IRA,
■ self-employed retirement,
■ Social Security benefits,
■ government retirement, including military plans,
■ governmental disability, including military plans.
Note: If your U.S. 1040 or 1040-SR Lines 4b, 5b, and 6b do not clearly identify the Social Security benefits and
retirement income you are reporting on Line 5, you must attach a copy of Form 1099-R or Form SSA-1099, Social
Security Benefit Statement, as applicable.

 

Turbtax seems to include the Federal 1040 and schedules 1, 2, and 3 by default but I can find  nothing in the Illinois instructions that requires this except in the case of ine 5
