On your Georgia return, the disability questions begin right after you state that you lived in Georgia all year and that you are not a first-time filer.

The next question will ask you about each member of your family.  The question is if a disability kept you from working.  You will answer Yes. 

Then, in the following page, is the area you were looking for.  You will enter when you became permanently disabled and the type of disability.
