Missing AZ State Form 322

The Arizona State download is missing Form 322, Contributions to Public Schools. Since AZ offers a dollar-for-dollar credit on these contributions, it is essential I have it available before filing. Does anyone know when this form will be added?

In the Arizona state tax return, at the screen School Tax Credits, select I donated to an Arizona public school by April 15 2016.
