When mailing state tax returns should I staple a copy of w2 and last check stubs (if filing form 4852 with Federal) even if the instructions do not say?

Yes, you should attach a copy of all income documents, such as W-2 forms, that show state tax withholding. However, you should send your last check stubs to related to form 4852. This is not required documentation related to this form.
