Arizona doesn't mail out your 1099-G. You have to go here to download a copy for your tax return.

You can enter that by clicking Federal

Wages & Income

Scroll down to "Other Common Income" 

(you may need to click "All income" first) 

Click Show more and scroll down to "Refunds Received for State/Local Tax Returns" 

Click Start and go through the interview

You do not need the paper 1099-G
