For NJ, do I add Cafe 125 HSA contributions to this form or am I double taxing myself? Does TurboTax already include my HSA contributions in my state return?

It depends on whether the box 14 code was correctly transferred to your state form. Each state is different, and some codes do not transfer over to the form to add to income. NJ is a state that does tax 125 contributions. If Box 16 on your W-2 shows the same amount as Box 3, the contribution has already been added to your wages and does not need to be entered in this section.
