I moved from MA to FL and forgot to change my tax state. MA correctly withheld 5,640, I should owe zero, as FL has no income tax and my income was not generated from a MA employer.
Is there a MA form to file?

You will have to file a Massachusetts nonresident or Part-Year Resident tax return and report zero state wages to get your withholding refunded.
Sounds good, I did prepare that form but did not zero out my wages…
