Lost 1099R. Is 2018 1099R online at IRS? Or must I file extension now and request copy of 2018 1099R right? Any penalties if I file for extension in time?

You get the 1099-R from the retirement plan administrator. It might be on their website.

There are no penalties if you are due a refund.
