None of the options you mention are listed. There are two options with the same title: Retirement distributions from: The first option includes New York State and City Education. The second option includes New York State and City Education (optional retirement program). So which do I choose?

 

After I submitted the question, I went back and clicked on Learn more on top of the page. As I drilled down, I saw that the New York Teachers' Retirement Program and 403(b) are included in the first option.

Generally, this question refers to indicating that your pension/annuity is from the military or from a state or local government or a state teachers plan or whatever.  The answer is used to determine if the distribution is nontaxable, partially, or fully taxable on the state level.

 

There should be an option for New York State and City Education that she should select.   This category includes:

 

New York State Teachers' Retirement System
New York City Teachers' Retirement System 
New York City Teachers' Retirement IRC 403(b) plan
International Union of Operating Engineers Local 891 Annuity Fund (Dept. of Educ. of NYC School District)

 

The TDA Program is an optional investment plan open to all TRS members - distributions are handled the same.
