Why do I have a code c in box 12 when my life insurance is 30,000?

Box 12a has code C 2.37. It is my understanding code c is for life insurance above $50,000, but my life insurance is $30,000. I should also note my amount in box 1 is drastically different than my amount in box 16. Thank you so much for any insight you are able to provide.
