Where is the substitute 1099R form

I took out all of my 401k and they never sent me a 1099R form so I looking for the substitute 1099R form.

Have you tried contacting the financial institution that runs the retirement plan to get the 1099R--or have you gone on their web site to see if you can print it from the site?
