Where do I include my K-94 form?

You need to review your Kansas state return and look for the screen that says Take a look at Kansas  credits and Taxes. On that screen choose Other Credits, then Other Credits and Credit carryovers. Under the Credit Information screen, choose Kansas Pregnancy Resource Center. Answer the questions in that section to generate Form K-94.
