YES! Thank you! I just finally got a call back from them and you are correct. Why that number is not on the form is beyond me.

I need payer I.D. for the states of MO and NE from Form 1099 G

It depends on what the 1099-G is for and should be listed on the form. They are easy to miss. Is it for unemployment?
