IF this distribution was from an employer sponsored retirement account then it would be entered on Form 1040 Line 5a from the gross distribution and Line 5b for the taxable distribution.

Go back to the Form 1099-R that you entered and edit the form.  Make sure that you did NOT check the box labeled IRA/SEP/SIMPLE
