where can i find statement of benefits on retirement income?

Notice from IRS asks me to send statement of benefits on retirement income but I don't see any tax form provided by my 401k company. Where can i find this information?

Are you asking about your 1099R? If you have been receiving retirement benefits, then the financial institution that manages your 401k sends you a 1099R in late January/early to mid-February. Or you might need to go on their website to get a copy of your 1099R.
