Early distribution penalty retirement account

My financial institution withheld penalties for early distribution, why doesn't it show on my 1099-R?

Was it the tax withholding for the tax 10% early distribution penalty?  That is included in the withholding in box 4.

OR just a plan early surrender fee?  That you don't report or get back or get credit for.

You will need to call your financial institution and ask them why.
