Two 1099-R same company with different state listed

I have a 1099-R with Florida listed as the state and a 1099-R with Nevada listed. I moved mid year from Florida to Nevada (same company and account number)

My return was kicked back because of 1099-R error.

Should I combine both amounts and list as one 1099-R with both states listed?

Neither state have income tax, 

I’m assuming that’s why it was kicked back……

Two 1099-R same company with different state listed

Yes, you should combine your two Form 1099-R's and list as one with both of the state's information listed in Boxes 15-16.  Doing this will result in the income being reported correctly on your return while also getting you past this error message due to having two Form 1099-R's from the same company.
