You were correct to leave the 'type of disaster' blank. Uncheck the box 'do you want to pay tax on the entire distribution in 2018?'.
Continue through to the end of the section, leaving everything blank (no $0's), except to answer NO to 'did you take a disaster distribution in 2017' (you may get the same questions for 2016, and if married, for both you and your spouse).
This (although somewhat confusing) is the IRS implementing the Disaster Relief Bill special considerations for Disaster Distributions.
