This distribution was for 2 IRAS that are in my name. One is with Ameriprise and this one IS with GoldStar Trust. I took it all from Golstar?

The Ameriprise amount should have been $3259 and the Goldstar $3660.64  hence the total $6919.94 which all came from GOLDSTAR?

If you believe the Form 1099-R isn't correct then you will have to contact your financial institute. 

Please follow these steps to enter your Form 1099-R:

1. Click on "Search" on the top right and type “1099-R”  
2. Click on “Jump to 1099-R”

If you had required minimum distribution from the two IRAs and you chose to make the total distribution from one IRA that is allowed... So when you get to the part where it asks was the distribution the rmd just say yes and click continue.
