WHY DOES 1099-R INDICATE IT NEEDS REVIEW?

First, if Box 2a is empty or says Unknown, leave it blank. Do not enter a zero there.

Also check to make sure you didn't type a comma somewhere instead of a decimal, or vice-versa.
