PUA-1099G does not have numbered boxes. Where do I enter the info requested?

Turbo tax asks for numbered boxes to be entered.
There are no numbered boxes

PUA-1099G does not have numbered boxes. Where do I enter the info requested?

If there are no box numbers then go by the description on the PUA-1099G for each box when inputting the form into TurboTax.
