I received a 1099-R form from my company. I have not made any withdraws from this retirement fund. Do I need to report this? Why did my company send two 1099R forms?

First, yes you do have to report both of those forms. I recommend that you contact your company to determine what is happening. It is possible that they rolled the account to a different investment manager. That could generate a 1099-R.
