Do I have to complete boxes 8-11 if a trad. IRA?

This has nothing to do with a Roth IRA.

Do I have to complete boxes 8-11 if a trad. IRA?

Assuming you are referring to a Form 1099-R, you should enter into TurboTax exactly as yours appears. If you do not have anything in boxes 8-11 on your form then you do not need to enter anything for those boxes in TurboTax.
