I moved and my previous landlord gave me my old mail. I recently found out that my previous employer closed my retirement account. I received a check from Fidelity for $512.08 dated June 27th 2016. I noticed the check was voided after 180 days. Because I just received it, I'm not able to deposit the check since it's voided. Also found a 1099R sent by Fidelity after I filed my taxes. It showed that the Federal tax withheld was about $132.49 and State tax withheld was $17.89.
I feel blindsided with this information since I was solely focused on filing my taxes with my current employer's W-2. I didn't expect this to occur. What should I do? Should I file a tax return amendment and ask Fidelity to reissue the check? What if I don't want to do anything with the check?

You will need to file an amended return in order to include the information from the Form 1099-R. If you do not amend, you will most likely be contacted by the IRS for not reporting the income since the Form 1099-R was issued both to you and the IRS.
Second, you should contact Fidelity to request that they re-issue the check. Timewise, it is too late to rollover the money into another account to avoid reporting the distribution. However, you could discuss putting the money into an IRA instead of simply cashing the check. If you did contribute the money to an IRA, that would be reported on your 2017 tax return.
Before starting to amend to include the Form 1099-R, you should wait to see if your return has been accepted or rejected by the IRS. If it is accepted, then you can go ahead with the amended return. If it is rejected, simply include the additional income before you resubmit your return.
Additionally, I would recommend that you wait until your original return(s) have been processed and you have received your refunds before filing the amended return. This will also allow more time just in case you still have some tax forms coming to you.
