Even if your 1099-R indicates that your retirement income is exempt from state taxes, you may still need to file a state tax return. Here are some key points:

what state? if NC, then yes. 

 

most states begin the tax return with federal adjusted gross income or federal taxable income.  

 

The pension will be deducted from those starting figures on the state income tax form.  So it has to be reported.
