On a 1099 G form on the part where it says payer do you like Employment Development Department or do you write unemployment insurance integrity and accounting

You use either one for the payor's name as both are right. The IRS will match the unemployment provider by the Payer's Tax Identification Number listed on Form 1099-G. Just make sure and type that number in and you will be fine. Please view the (screenshot) below for reference.
