My1099-R form info has Fidelity Investments listed twice. I only have one Fidelity account and one 1099-R form for 2018. Do I have to enter the same info twice?

No delete one of the 1099R's as if one 1099R and have same code in box 7 and same $$ on it, then it should be entered once.
