That's right. There is no box 12a on a Form 1099-R. Why do you think there should be a box 12a? Maybe you are entering your 1099-R in the wrong place. Are you entering it as a W-2 instead of as a 1099-R?

Box 12 is state tax withheld. If your 1099-R does not have state tax withheld in box 12 then leave all boxes 12-14 blank.
